For Emergency Medical Services (EMS) report requests, please visit the Carroll County Department of Fire/EMS report request webpage. 

To request a copy of a Fire/Rescue report, please read the following:

• Requests for reports MUST be made in writing
• Must include the name, address and contact name of the requestor and must state their involvement. (attorneys, insurance companies, patient, parent/guardian, owner)
• Must include the type of call, date, time, Incident address, name/DOB of patient if applicable and incident number if available.
• EMS reports requests will need to include a release of information from the patient and/or guardian.

Costs

• Subpoenas will be honored and will not cost anything.
• Requests for itemized billing only will not be charged.
• All other reports are $25.00. Checks should be payable to the Gamber & Community Fire Company.

Once all the required documentation and check is received:

• Subpoenas, EMS/Fire/Rescue reports, and non-itemized billing information will be mailed, emailed or faxed to the requestor.
• Itemized billing information will be forwarded to the Billing Company authorizing them to release the report and itemized bill.
